Fires on board vessels

It has been brought to our attention that the hazard Ineffective communications: Confined space originally had two control measures:

1. Effective communication (as per Incident command)
2. Establish effective communications (which related to confined spaces)

As the second control measure had been removed in error in both the Operational Guidance and the Training Specification, this has been corrected by adding the control measure Confined space: Supervision, resourcing and communication, which is published in the Height, structures and confined spaces guidance.
